Low Back pain: Why Movement Is Often the Best Medicine

Low back pain is one of the most common reasons people seek physiotherapy. Whether it starts after lifting something heavy, spending too long sitting or seemingly out of nowhere, back pain can make everyday activities uncomfortable and frustrating.

The good news? Most episodes of low back pain improve within a few weeks, and research shows that staying active is one of the best things you can do to support recovery.

What causes low back pain?

Low back pain can involve muscles, joints, discs and other structures around the spine. However, in most cases, there isn't one specific structure responsible for the pain.

Importantly, pain doesn't always mean damage. Your back is strong, adaptable and designed to move.

According to Australia's healthdirect, more than 90% of back pain cases are not caused by a serious medical problem.

What does the evidence recommend?

Australian clinical guidelines emphasise three key strategies for managing most episodes of low back pain:

1. Keep moving

Prolonged bed rest can slow recovery. Gentle walking, regular movement and gradually returning to normal activities are generally more beneficial than avoiding movement altogether.

2. Build strength and confidence

Individualised exercise, including strengthening, mobility work and clinical Pilates, can help improve function and confidence in movement. The goal is to gradually build your capacity to do the things you enjoy.

3. Understand your pain

An MRI or X-ray isn't always necessary. Changes seen on scans are common, even in people without pain. A thorough clinical assessment can help determine whether further investigation is needed.
